WELFARE FRAUD COMPLAINTS

  • The purpose of this form is to provide another avenue for the public to file a welfare fraud complaint.
  • Use this form to report a person or vendor (for example: land lord or childcare provider) that you believe has fraudulently received assistance from programs administered by Chautauqua County Department of Social Services.
  • If you have knowledge of anyone recieving money or services provided by Chautauqua County Department of Social Services and you believe that they are not entitled to the money or services please fill out the form below

The Department of Social Services Fraud and Recovery office can be contacted anytime between the hours of 9 a.m. and 5 p.m. in the winter and 8:30 a.m. to 4:30 p.m. in the summer.
