You can choose a Primary Care Physician (PCP) for your child, who will coordinate all care.
You can join Child Health Plus if:

Your Child is under age 19

Your child has limited or no health insurance and is not eligible for Medicaid

You and your child are New York State residents and live in one of the following counties: Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans or Wyoming.
