What is Child / Teen Health Plus?
The Child / Teen Health Program is a preventive health program which
provides continuing medical care to your children. Child / Teen Health
Plus makes use of a full range of health care services to detect health
problems before they become serious and hard to treat.
Who is eligible for Child / Teen Health Plus?
Every Medicaid recipient, from birth until age 21, is eligible.
Why Child / Teen Health Plus?
Child / Teen Health Plus is important because it provides a child with
early and regular health care. It assures a child of better health now
and fewer health problems in the future. Depending on the age of a child,
a Child / Teen Health Plus screening will include:

Health History

Hearing and Vision Testing
|
|
Complete Physical
|
Blood Tests (such as, blood lead level, sickle
cell anemia.)
|
|
Immunizations
|
Developmental Assessment
|
|
Dental Screenings
|
|
Your child's medical provider will arrange for Follow-up Treatment of
problems uncovered during the examination.

Where do you find Child / Teen Health Plus ?
- If you are enrolled in a managed care plan, the plan provides Child
/ Teen Health Plus.
- If you are not enrolled in a managed care plan, you can call your local
department of social services to locate a provider for your children.
