Program Listing & Description

CASE MANAGEMENT- CASA Registered Nurses can provide case management to individuals receiving home care services. Case management can help to ensure a person receives appropriate, effective and efficient services to meet their needs to promote independent living for as long as it is safe.


CERTIFIED HOME CARE offers nursing, home health aides, social work, occupational therapy, physical therapy, and dieticians. Payments may be made by Medicare, Medicaid, private insurance, and private pay.

CONSUMER DIRECTED PERSONAL ASSISTANCE PROGRAM is designed to help with personal care which allows those with Medicaid to hire and manage their own care provider. This program requires a physician's order and is for those with a medical disability.

PERSONAL CARE / HOUSEKEEPING SERVICES is for those with medical disabilities that require assistance with personal care, meal preparation, light housekeeping, laundry, and shopping. This program will require a home visit assessment by a CASA nurse to discuss payment options. For those on Medicaid, a physician's order is required before a home visit can be made. CASA contracts with a number of local licensed agencies to provide this service.

PRI & SCREEN - is an assessment required by New York State before admission into a nursing home. This screening must be performed by a Licensed Professional or Registered Nurse that is certified by New York State to administer the PRI & Screen. This service is offered FREE of charge through CASA.

PRIVATE DURY NURSING - CASA Nurses with assess for the need of hourly RN and LPN services in the home for those with Medicaid. A physician's order is needed for this assessment and service.

RESPITE - Service that will provide care to an elderly person and those with medical disabilities to give the caregiver time to do something other than caregiving. This service may be supplied in the home or in an institutional setting. CASA Nurses assist with obtaining respite care.

WAIVER SERVICES - Home and Community Based Waivers provide community support services to those Medicaid beneficiaries who would otherwise require institutional care. For more information about waiver services....
