Listed below are several commonly used employer forms issued by the Chautauqua County Department of Human Resources. Application for Examination or Employment
On form
Application form for examinations and appointments. This form must be printed out, completed and delivered to our Mayville New York office.
New Position Duties Statement Prepared by an appointing authority requesting the creation of a new position. The MS word version allows the user to complete most form items using a pc. The form must be printed for signature and delivered to our Mayville New York office.
Report of Personnel Change Form (County Government) Used by County Government Departments or Agencies to report new employees and changes for existing employees.
Report of Personnel Change Form (Municipalities, Schools and Special Districts) Used by Municipalities, Schools and Special Districts to report new employees and changes for existing employees. The MS word version allows the user to complete most form items using a pc. The form should then be printed for signature and delivered to our Mayville New York office. The Adobe Acrobat version must be printed out, completed and delivered to our Mayville New York office. All supporting documents should be attached to the RPC.
