Chautauqua County Chemical Dependency Services is an outpatient
alcohol and substance abuse clinic serving Chautauqua County residents
who have a chemical dependency problem or who have a family member with
a chemical dependency problem.
Services Provided:
Services provided include evaluation, referral,
and outpatient treatment. Outpatient treatment generally includes a combination
of treatment modalities such as individual counseling, group counseling,
and intensive outpatient treatment.
MICA - Offering life skills development for individuals experiencing both
substance abuse and mental health problems
YOUTH - The purpose of the Adolescent program is to increase self-worth
while decreasing the need to alter self with the use of drugs and alcohol.
ADULT CHILDREN OF ADDICTED FAMILIES RELATIONSHIP RECOVERY - Assists family
members in identifying how addiction impacts their lives. The focus is
on positive support, healing, and change.
How to get services:
Call the clinic during normal working hours. Identify yourself as requesting
services and wanting to schedule an appointment. You will be asked some
questions so we can gather basic information, determine the reason you
are requesting services and how we can best meet your needs.
Confidentiality:
All services are confidential. The clinic follows state and federal rules
and regulations governing the confidentiality of client identifying information
and client records.
Fees:
Financial information is discussed during the initial phone contact. Intake
and clerical staff will review available coverage, will facilitate verification
of insurance coverage and provide pre-certification assistance. Insurance
plans and Medicaid are readily accepted for payment of services. Fees
are arranged according to ability to pay. A pre-determined fee scale is
used in determining your fee in conjunction with income verification materials.
No person will be refused services on the basis of income.

The clinic is closed for the following holidays: New Year's Day, Martin
Luther King Day, President's Day, Memorial Day, July 4th, Labor Day, Columbus
Day, Election Day, Veterans Day, Thanksgiving, Christmas.
